2019-20 Emergency Medical Authorization form due August 2
Loveland parents/guardians: It’s time to complete your child’s Emergency Medical Authorization (EMA) form for the 2019-2020 school year.
The EMA form is due no later than Friday, August 2, 2019. Students will not be able to receive classroom information, schedules or Chromebooks until the EMA form has been completed and submitted to the district.
The EMA form can be accessed via Tigerparent.me (https://tigerparent.me) – the one-stop-shop communication portal where parents will be able to view all of their students’ classroom and building information once the school year begins. Please make sure you have an account set up (Tigerparent.me Getting Started Guide). The EMA form log in is also accessible via www.lovelandschools.org. Click on the “Parents” tab in the upper right corner and find the Emergency Medical Authorization Form in the listing.
Please keep in mind that all information on the EMA form should be kept up to date throughout the school year. The district uses the information to contact you in case of an emergency. All changes have to be made by parents; staff members are not permitted to make changes to the data on the forms, including phone, email or any other information.
Please note that if you are new to Loveland and enrolled your student for the 2019-20 school year between January 2019 and today, you are also required to follow this procedure.
If you forgot your log in username and/or password, click on “forgot credentials” and the information will be emailed to you. In case you do not have access to a computer, please contact your building secretary to arrange a time to complete the form in the school office.
