IMPORTANT REMINDER: Please review Emergency Medical Authorization forms

Congratulations on reaching the halfway point through this school year! We know as parents you stay busy with helping your child with homework, dinner and running them to after-school activities. We don’t want to burden you with another task and we wouldn’t ask if it wasn’t important, but we need your help. It is imperative for your child’s safety that we have the most up to date contact information on file for them.
Please take a moment to log into the Emergency Medical Authorization (EMA) forms for each student in your house to review the information we have on file for them. Click here to access the forms. Once there, please make corrections and/or additions to any phone numbers, contact preferences or emergency contacts.
Please note: In order to receive alert notifications you must have at least one phone number checked next to “use for emergency calls”. New this year – you may add notes for contact preferences (examples would include “call mom first” or “call dad at work”).
We would also like to ask that you separate each individual’s name if they appear jointly under Parent/Guardian #1. This will help us better organize your information and be sure we are contacting the correct individuals at all times.
When you are all done, please be sure to click on “Save and Submit” at the bottom of the page.
