It's time to complete your child's Emergency Medical Authorization (EMA) form for the 2018-2019 school year. If you have already completed this form, thank you! All EMA forms are due no later than August 3, 2018.
AGAIN THIS YEAR: Students will not be able to receive classroom information or student schedules until the EMA form has been completed and submitted to the district. If you do not have access to a computer, please contact your building secretary to arrange a time to meet at the school office for assistance. Click here to complete the EMA form,
or go to lovelandschools.org, click on the word "Parents" on the top right of the page, then click on "Emergency Medical Authorization Form." Keep in mind that all information on the EMA form should be kept up to date throughout the school year. This is the information that we use in order to contact you in case of an emergency. All changes will have to be made by parents. Staff will not be permitted to make changes to the data on the forms, including phone, email, or any contact information.
If you have forgotten your username and/or your password, click on "forgot credentials" and the information will be emailed to you.
If you are new to Loveland and have enrolled for the 18-19 school year anytime between January of 2018 and today, you will also have to follow this procedure.
