IMPORTANT INFORMATION: EMA Forms
It’s time to complete your child’s Emergency Medical Authorization (EMA) form for the
2017-2018 school year. Please note: This is a separate form submission from the one required by the Athletic Department.
All EMA forms are due no later than Friday, August 4, 2017.
Students will not be able to receive classroom information, student schedules or Chromebooks until the EMA form has been completed and submitted to the district. If you do not have access to a computer, please contact your building secretary to arrange a time to meet her at the school office for assistance.

Keep in mind that all information on the EMA form should be kept up to date throughout the school year. This is the information that we use in order to contact you in case of an emergency. All changes will have to be made by parents. Staff will not be permitted to make changes to the data on the forms, including phone, email or any contact information.
If you are new to Loveland and have enrolled for the 2017-18 school year anytime between January 2017 and today, you will also have to follow this procedure.
